Integrated Community Health
Description
This project seeks to strengthen community capacity for preventive health by training community members and local leaders in basic health education, screening support, referrals, and early detection, supported by practical demonstrations from health and welfare professionals. It responds to a community‑identified priority for early detection of common health conditions, improved health awareness, and easier access to social protection services, including support for registration under the National Health Insurance Scheme. Health professionals will provide screening services and demonstrations on hypertension, diabetes, hepatitis B, and vision‑related conditions, along with targeted education on preventive healthcare, reproductive health, and healthy lifestyles.
Social welfare officers will help community members access welfare services through information sharing and registration assistance for vulnerable households. Community volunteers and selected leaders will be trained in health education, screening support, referral systems, data recording, and mobilization skills that equip them to assist outreach activities and maintain preventive health messaging after the project concludes. Expected outcomes include improved health‑seeking behaviour, increased screening services, greater awareness of available welfare services, and strengthened systems for ongoing outreach and health promotion.
Sustainability will be supported through community ownership and the use of existing local structures. Community health personnel and welfare committees established through the project will continue mobilization and health education beyond the grant period. Trained volunteers will also support health staff and help mobilize community members for ongoing health awareness and referral support.
