A health home is a place where you will receive all of your primary medical care, dental and wellness needs. We are an integrated health home who coordinates comprehensive care for all of our patients regardless of their ability to pay. We have served San Benito and the surrounding counties since 1975 and were designated as a Federally Funded Community Health Center in July 2007. The Foundation offers Medical, Dental, Behavioral Health, Comprehensive Perinatal Service Program (CPSP) and the supplemental nutrition program Woman, Infants and Children (WIC). San Benito Health Foundation offers preventative services meant to keep you and your whole family healthy.

Duties

Great opportunity for a motivated FNP looking to make a difference at the community level, and work with a team in an outpatient setting to a multicultural patient population and help patients to achieve the best level of wellness. Candidates must have a current California FNP license. New graduates are welcome to apply. Individuals will work in a Patient Centered Medical Home Model, and work closely with a team of professionals to provide culturally sensitive, high quality, comprehensive health services in our Federally Funded Community/Migrant Health Center. Candidates must have the ability to work independently and must have valid Driver’s license and be insurable.

FNP provides quality care to all of the patients at SBHF, participates in quality improvement, prevention activities in the community and health disparity reduction projects, and Collaborates with those responsible for case management, social services, referrals, outreach, community prevention and other SBHF managers.

Qualifications

Applicants must meet the following criteria:
NP Degree from a recognized University
1-2 years’ experience working in a primary care clinic setting preferred
Current and Valid NP License
Current and Valid DEA License
Bilingual/Spanish is a must
