About the organization

The mission of Cayenne Wellness Center is to increase the quality of life for individuals diagnosed with sickle cell disease in California by ensuring expert, unbiased, and comprehensive care.

Duties

Under the direction of the Executive Director of Cayenne Wellness Center, the Community Health Worker (CHW), will work on behalf of various grants to:
provide health education and health promotion using evidence-based information for SCD and related issues;
develop partnerships with family organizations to ensure that families are empowered as partners in their care;
assist families with obtaining SCD educational and social support services;
assist with transition services; and
engage in activities that help ensure individuals with SCD have access to services, including a medical home, from the time of diagnosis through the lifespan.

The CHW will be responsible for assisting persons diagnosed with SCD to become advocates through empowerment. CHWs will do so by utilizing Cayenne Wellness Center databases, working with hospital and medical centers, state agencies that provide newborn screening follow-up, hospitals, schools, churches, and other CBOs that provide services to patients with SCD.

CORE DUTIES:
Identify persons diagnosed with Sickle Cell Disease (at minimum three persons/month) and coordinate access to a medical home within two months
Provide all social support services as needed (i.e., transportation, counseling, insurance, and housing).
Educate patients on the latest FDA SCD modifying therapies
Build partnerships with various entities in designated regions hospitals, schools, churches, state agencies,

Qualifications

• Knowledge of basic medical and sickle cell terminology, communities being served and available resources, and confidentiality standards and HIPAA compliance standards
• Skilled in written, verbal, and interpersonal communication, time management, multi-tasking, managing multiple projects, and excel and shared computer applications
• Able to work within a team and with peers and with minimal supervision and independently
• Able to navigate the computer, create spreadsheets, and shared computer
applications
• Ability to prioritize and meet deadlines

Minimum Qualifications (REQUIRED):
• Minimum B.A. degree required
• Minimum of 2 years experience in health, social, or community services required
• Proof of CA ID (driver’s license; Proof of auto insurance required) to run tasks

Preferred Qualifications:
• Bilingual Spanish-English preferred
• Previous experience working in/with local CBOs or agencies.

Salary

$17.00 per hour
