About the organization

United Way of East Central Iowa (UWECI) works with more than 100 cross sector partners to fight for the education, financial stability, and health of every member of our community. My Care Community is a care-coordination system built to provide service providers and clients with more effective and efficient processes to help the clients access services to improve their education, financial status, or physical or mental health.

This project is part of the Community Corps VISTA Program, operated by Iowa & Minnesota Campus Compact. The program strives to eliminate poverty through campus-community partnerships by placing full-time capacity building support in nonprofit organizations and higher education institutions. There are three focus areas, this project being in the Health Corps, which supports sites in long-term approaches to the social determinants of health. This includes meeting low-income student needs and a variety of projects with health-focused community organizations.

Duties

Member Duties: The My Care Community VISTA will be working with agencies and programs to align their processes with the MCC system, providing them with individualized manuals and trainings to garner their staff’s buy-in and consistent use of the MCC platform. This VISTA will work with a variety of organizations across the community to build a robust care-coordination system in our community.

As a member of My Care Community (MCC), UWECI and the VISTA will provide standardized procedures and trainings across the more than 25 current MCC partners and onboard new programs to increase the effectiveness of the system. The VISTA member will develop a standard training module that can be adapted to the needs of each organization/program, adapt a user manual for each new partner, and provide initial training for staff members.
