Copies of the following Peace Corps records are available to all current and former Volunteers who have completed the initial training for Peace Corps service.

Description of Service Statement

The DOS is an official account of your responsibilities, duties, and accomplishments while serving as a Peace Corps Volunteer. This document is housed at Peace Corps Headquarters for 60 years from your end of service date. Copies are available by written request to Office of Volunteer & PSC Financial Services, Peace Corps,  Attn: Volunteer Certification Specialist, 1111 20th Street, NW, 2nd Floor, Washington, DC, 20526. Verification of Service

Verification of service dates are available by written request to Office of Volunteer & PSC Financial Services, Peace Corps, Attn: Volunteer Certification Specialist, 1111 20th Street, NW, 2nd Floor, Washington, DC, 20526. Volunteer/Personnel Payroll File

This file contains all documentation relative to your readjustment allowance and deductions. These records are housed at a Federal Records Center, and are destroyed six years after your COS/Termination date. Copies are available through written request to the Office of Volunteer and PSC Financial Services, Peace Corps, 1111 20th Street, NW, 2nd Floor, Washington, DC 20526. Medical File

This file contains a record of medical exams required for Peace Corps placement, record of immunizations, dental records, etc. Medical files are housed at a Federal Records Center or at the Office of Medical Services, and are destroyed 25 years from your termination of service date. All requests for these records must be submitted in writing to the Office of Medical Services, Peace Corps, 1111 20th Street, NW, Room 6400, Washington, DC, 20526.  The phone number is 855.855.1961, ext. 1500.

Country File

This file is maintained in country for only one year after your end of service date. It may take up to two months to receive your country file, because requests are sent overseas and pouched to Headquarters. Copies of the country file are available by written request to the Privacy Act Officer, Office of Management, Peace Corps, 1111 20th Street, NW, 2nd Floor, Washington, DC, 20526. The phone number is 855.855.1961, ext. 1186.

Applicant File

The applicant file contains all documents collected as a result of your application for Peace Corps service. It includes your original application, reference forms, recruiter's comments, background information form, etc. These files are housed at a Federal Records Center and are destroyed four years from your reporting-to-training date. Send a written request to: Privacy Act Officer, Office of Management, Peace Corps, 1111 20th Street, NW, 2nd Floor, Washington, DC, 20526. The phone number is 855.855.1961, ext. 1186.

Note: All of the above records are subject to privacy act restrictions. Denial of all or part of a record to a request must be processed through the Office of Management for review of the record and release to the individual in accordance with the privacy act.
